Membership to the Florida Allergy, Asthma & Immunology Society is open to Florida Licensed Physicians in good standing. Membership shall be limited to those physicians who have completed an allergy and immunology fellowship accredited by the American Council for Graduate Medical Education (ACGME) or those who are currently in an ACGME-accredited allergy and immunology training program.

1. Please download the Membership Application Form and send via email to faais@faais.org. Print and complete by hand or download and open using Adobe Acrobat for fillable form fields.
2. Click the “Pay Dues” button to pay the membership fee. Under membership type, please select “Member Applicant.”
